Health Care Proxy

Who Will Speak for You?

In New York State, there exists a law called the New York Health Care Proxy law. This law allows you to appoint someone you trust, such as a family member or a close friend, to decide about medical treatment if you lose the ability to decide for yourself.

Why is a Health Care Proxy Important?

If you ever become unable to communicate with the doctors or any other medical staff involved in your care, without a health care proxy, neither a family member nor a close friend will be allowed to make decisions about your treatment, even if they know what you would choose. Completing a health care proxy empowers a trusted individual to carry out your wishes regarding your medical treatment.

Where Can I Obtain a New York State Health Care Proxy Form?

Once you decide who you wish to be your health care proxy (agent), you will be able to complete the New York Health Care Proxy form. The form, including instructions, is available in the following languages: English, Spanish, Chinese, Haitian Creole, Korean, and Russian.
