Division of Community and Population Health
In January 2017, Wyckoff formed the Division of Community and Population Health to further Wyckoff’s mission to provide quality care, with kindness and respect.
Objectives
The Division of Community and Population Health:
- Promotes and increases public awareness of Wyckoff Heights Medical Center and the services we provide to the community.
- Engages the community to identify and prioritize needs and develops and implements services that will help people achieve and sustain good health.
- Works across multiple departments and community sectors to improve health and health equity, and to address environmental, cultural, and social determinants of health specific to the neighborhoods we serve.
- Operates innovative special projects to address population-specific needs, such as those of our elders, children and families, persons with different abilities, and persons living with chronic conditions.
- Collaborates with community members, community based organizations, and safety net services, to implement Wyckoff’s Community Service Plan.
- Engages in public policy and government relations work to advocate for community needs and priorities.

PROMOTING A HEALTHY AND SAFE ENVIRONMENT
Violence Intervention and Treatment Program Counseling and services for survivors of violence and sexual assault, and violence prevention education services for the community, including Rape Crisis and Sexual Assault Forensic Examination Services.

PROMOTING HEALTHY WOMEN, INFANTS AND CHILDREN
PROMOTING MENTAL HEALTH AND PREVENTING SUBSTANCE ABUSE
PREVENTING HIV AND HEPATITIS C AND PROMOTING SEXUAL HEALTH
Positive Health Management Provides HIV, STD, and Hepatitis C Prevention and Care services, including Biomedical Prevention (PrEP). A welcoming community-based site includes Comprehensive HIV Primary Care as well as Behavioral Health, Case Management, and other Supportive Services. Many services are available for the uninsured.
